In timing belt driven engines, a tensioner keeps constant pressure on the timing belt and ensures the belt doesn't slip. These tensioners use a spring-loaded (and sometimes hydraulically dampened) tensioner assembly to put pressure on a pulley/roller, which in turn presses on the belt itself.
When servicing the timing belt, it's important to replace the tensioner and the roller/pulley at the same time, as the spring in the tensioner can gradually weaken over time, the hydraulic fluid can leak out of the damper, or the bearings inside the pulley can wear out and make the roller seize up.
